Description: DEPARTMENT\: Advanced Practice NursingSUPERVISOR'S TITLE\:Clinical Director, Business Director, Department Director COLLABORATIVE RELATIONSHIPS\: All members of the interdisciplinary healthcare teamTITLES SUPERVISED\:N/ATOTAL NUMBER OF FTEs DIRECTLY SUPERVISED\: 0TOTAL NUMBER OF FTEs INDIRECTLY SUPERVISED\: 0PURPOSE OF POSITION\: To work collaboratively with families and a multidisciplinary health team to provide primary care or acute/chronic health care to pediatric patients, including clinical management for a specialty population of a Medical Center dedicated to the care of infants, children, adolescents and some adult patients. Subspecialties with Inpatient service coverage or combination inpatient/outpatient. Inpatient responsibilities include management of patients in an inpatient unit. Treatments and procedures are administered in outpatient and/or day hospital setting. May require evening, and weekend hours and call responsibility but does not require 24/7 coverage responsibility. Provide leadership through education, consultation and research. Responsibilities·Interprofessional Practice\:Demonstrates consistent integration of the Interprofessional Practice Model (IPM) in all aspects of practice.·ComplianceSustain a working understanding of regulatory requirements and accreditation standards. Support leadership in efforts to meet and sustain the regulatory compliance efforts of the department and hospital. Be mindful and judicious in monitoring safety and departmental policies and procedures. Ensure all individual certifications and Safety College requirements are fulfilled in a timely basis. Work with department managers, nurse council and magnet representatives to identify and share safety best practices. Be mindful of CCHMC processes and compliance with CMS standards and freely communicate safety and compliance concerns to leadership team members.·Patient Care-Age Specific and Culturally DiverseConsistently integrates age specific age specific and culturally diverse concepts into patient care, taking into consideration both the patient's chronological age and developmental functioning.·Assessment and DiagnosisCollects & documents comprehensive data in a systematic & ongoing process pertinent to the patient's health or situation, involving the patient/family caregiver when appropriate; Initiates & interprets age-appropriate & condition-specific laboratory tests & diagnostic procedures; Analyzes & prioritizes complex data to determine the differential diagnoses & revises diagnoses as appropriate to initiate the plan of care; Assessment & diagnoses derived are developmentally appropriate & specific to areas of growth & development, age, cultural sensitivity, & family dynamics.·Outcomes Identification and PlanningDevelops outcomes as measurable goals that provide direction for continuity of care & continuity of care & consistency among providers. Develops, prioritizes, evaluates & modifies an individualized comprehensive plan of care. Addresses each diagnosis or issue, as appropriate. Derives outcomes & plans of care that involve the patient/family/caregiver when possible & are realistic to the patient/family/caregiver capabilities & resources. Considers risks/benefits, costs, clinical effectiveness, current scientific evidence/clinical guidelines & clinical expertise to formulate interventions, treatments & outcomes. Incorporates new knowledge & strategies to initiate change in practice if desired outcomes not achieved. Maintains documentation & updates accordingly outcomes, goals & the plan of care.·Care Management, Education, and TreatmentMaintains competency in skills/procedures; Promotes self-management & family centered care; Employs diverse & complex strategies, interventions, & teaching to promote health & a safe environment; Provides leadership in care management to achieve optimal quality, cost-effective care; Documents & communicates with healthcare team members to achieve an integrated delivery of pediatric care services; Delegates appropriate monitoring, assessment, & interventions according to the patient and scope of practice of the caregiver; Provides consultation to influence the identified plan of care, to enhance the abilities of others to provide health care, and to effect change in the healthcare system; Makes appropriate referrals and discusses recommendations with patient/family/caregiver; Orders and/or performs appropriate treatments, therapies, & procedures that are based on current knowledge, research, & practice; Provides patient/family/caregiver with diagnostic & laboratory results as well as actual/potential benefits, limitations, & adverse effects of proposed therapies.·Professional PracticeParticipates in quality improvement activities or research to improve the care delivery process & patient care outcomes; Uses & communicates results to initiate change in practice; Uses creativity, new knowledge, & innovation to improve care; Takes action to achieve goals identified during performance appraisal & peer review, resulting in changes in practice & role performance; Participates in lifelong learning that improves skills & competence in clinical practice/role performance; Serves as preceptor, role model, or mentor; Contributes to the professional development of peers/colleagues to improve pediatric health care & to foster the profession's growth; Integrates ethical considerations & research findings into practice; Provides leadership by participating on committees or in professional organizations, or writing/publishing/presenting. Advocates for the pediatric patient/family/caregiver; Serves as a leader, influencing both healthcare/APN practice & policy.·Prescriptive AuthorityPrescribes appropriate pharmacologic/non-pharmacological agents & interventions that are both universal & unique to the patient, & monitors/evaluates effects.·EvaluationUses a systematic, ongoing evaluation of the assessment data & outcomes to revise or resolve the diagnoses, outcomes, the plan of care, & the implementation as needed; Evaluates the accuracy of the diagnosis, effectiveness of the interventions & the plan of care strategies in relation to attainment of expected outcomes; Includes the patient/family/caregiver & other healthcare providers; Bases the evaluation process on advanced knowledge, practice, research, & documents appropriately; Utilizes the results of the evaluation to determine the impact on the patient/family/caregiver, organization, & system to make/recommend process or structural changes including policy, procedure, or protocols as appropriate. Qualification: Skills & Competencies ·Clinical Care Skills Skill in physical assessment, diagnosis, well-child care, and management of illness ·Development Knowledge of growth and development ·Family-Centered Care Understands and supports family-centered care ·Clinical Knowledge - Specialty Professional knowledge and clinical ability sufficient to provide evaluation and treatment of complex patients in one or more specialty areas ·Diversity Appreciation Understanding and showing respect and appreciation for the uniqueness of all individuals; leveraging differences in others' perspectives and ideas; appreciating cultural differences and adjusting one's approach to successfully integrate with others who are different from oneself ·Other Demonstrate skill in leadership, consultation, research and teaching ·Communication-Advanced Excellent verbal, written and/or interpersonal communication skills ·Specialty Knowledge Knowledge of pathophysiology and pharmacology Required\: ·Master's Degree in Nursing AND Current license to practice nursing in the State of Ohio ·Certification as an advanced practice nurse appropriate to meet requirements of Nurse Practitioner position (e.g., PNP or FNP) required AND Certificate to prescribe as applicable ·Minimum 2 years pediatric nursing experience with emphasis in area of specialty Must have one year at least as NP Preferred\: Prefer more experience as NP Prefer critical care background
