HOME HEALTH Job Description: We are seeking a dedicated and
compassionate Physical Therapist to join our dynamic team of
healthcare professionals. As a Physical Therapist, you will play a
crucial role in evaluating, diagnosing, and developing personalized
treatment plans to improve patients' physical function, mobility,
and overall quality of life. This position is ideal for an
individual who is passionate about patient care and thrives in a
collaborative environment. Key Responsibilities: - Conduct thorough
assessments of patients' physical conditions and functional
limitations. - Develop individualized treatment plans based on
patient evaluations and goals. - Implement therapeutic exercise
programs, manual therapy techniques, and modalities to enhance
physical performance. - Educate patients and their families on
treatment plans, exercises, and self-care strategies to support
recovery and prevent injury. - Collaborate with a multidisciplinary
team, including physicians, occupational therapists, and other
healthcare professionals, to coordinate comprehensive patient care.
- Monitor, document, and evaluate patients' progress and adjust
treatment plans as necessary. - Maintain accurate, up-to-date
patient records in compliance with regulatory standards and
facility protocols. - Stay current with advancements in physical
therapy techniques and integrate evidence-based practices into
patient care. - Engage in continuous professional development
through workshops, seminars, and other educational opportunities.
Qualifications: - Master's or Doctoral degree in Physical Therapy
from an accredited program. - Valid state licensure or eligibility
for licensure to practice as a Physical Therapist. - Strong
knowledge of physical therapy treatment approaches and techniques.
- Excellent communication and interpersonal skills. - Ability to
effectively motivate and instruct patients of all ages and
abilities. - Strong organizational and time management skills. -
Commitment to maintaining patient confidentiality and delivering
high-quality care. - This a flexible Part-Time or PRN role . Join
