LPN Team Float Role: This is a DRIVING position and will travel to
see patient in both homes and facilities within Huber Heights OH
and surrounding areas This is a FT day shift position, 5-8hr shifts
per week with an every 6th weekend rotation and 1-2 holidays per
year We provide superior care and superior services to patients at
their end of life journey . Only those who have a heart for hospice
will succeed The LPN/RN Float Essential Duties: The LPN Team Float
provides quality nursing care to patients in a manner that meets
the patient and family needs working under the direction and
supervision of an RN Care Manager. Qualifications: LPN's -
Registered LPN with the State of Ohio without any board actions.
One year of previous nurse experience. CPR Certified Certification
in Hospice & Palliative Medicine (CHLPN) preferred; required within
two years of employment. (Eligible upon completion of 500 hours in
last 12 months of hospice/palliative experience.) Computer skills
sufficient to properly document services and care. Ability to drive
during daytime, nighttime or inclement weather. Valid Driver's
License with Safe Driving Record State Minimum Automobile Insurance
