Job Description:
Facility: Samaritan Behavioral Health, Inc
The Registered Nurse (RN) functions as a member of the medical
services team to assist in the provision of clinical care,
intervention, educational services and care coordination to child,
youth and adult patients. The RN collects data relevant to the
delivery and promotion of medical services. The RN partners with
and advocates for patients/families/significant others to provide
physical, emotional and spiritual support which impacts patient
outcomes in a safe environment. The RN functions in a versatile
role that demonstrates professionalism and promotes excellence and
autonomy in the practice of Nursing. The RN utilizes the nursing
process in conjunction with evidence-based principles to care for a
diverse caseload of patients within the department scope of
service. The RN serves as guide, healer, collaborator, teacher,
leader and sentry. The RN collaborates with and guides care
delivered by the inter-professional team. The RN is accountable for
compliance with the Ohio Nurse Practice Act, Nursing Code of Ethics
and applicable regulatory standards.
Schedule and provide direct face-to-face pharmacological and case
management services according to departmental standards and
productivity expectations. Refer patients for other services when
indicated.
- Collaborate and coordinate with referral source, internal team
members and external service providers to ensure appropriate client
care and continuity of care.
- Complete all direct service documentation within established
agency and departmental guidelines.
- Review and sign patient medical history forms. Make
recommendations to patients, appropriate staff and/or
physician.
- Provide other direct, face-to-face, services to clients according
to departmental standards and productivity expectations.
- Provide billable case management (CPST) and other medication
management services to coordinate clinical care in the form of face
to face, telephone calls, report writing and case conference
meetings.
- Demonstrate clinical knowledge and skills relevant to the
clinical services and intervention methods provided to clients.
- Manage medical orders, including calling in medications,
educating patients regarding proper use of medications, side
effects, and coordinating lab work according to departmental
standards.
- Seek consultations regarding medical needs of patients. Maintain
on-going communication with medical providers for the purpose of
coordinating and evaluating patient care.
- Demonstrate knowledge of SBHI and departmental medical records
and other paperwork standards. Apply competencies in both the
provision and documentation of medical records.
- Demonstrate knowledge of regulatory standards. Ensure that
clinical records are maintained in compliance with these standards
at all times.
- Respond to patient inquiries and collaborate with physicians
regarding patient information.
- Attend required staff meetings and trainings.
- Maintain Medication Log for samples sign-in and dispensing.
Submit the Medication Discrepancy Log on a monthly basis.
- Per medical provider order give injections as prescribed to
identified patients as applicable.
- Routinely provide medication side effect sheets to patients.
Education
Minimum Level of Education Required: Associate degree
Additional requirements:
- Type of degree: Graduation from an accredited school of
nursing
- Area of study or major: Nursing
- Preferred educational qualifications: BSN preferred
- Position specific testing requirement: N/A
Licensure/Certification/Registration
- Current licensure as a Registered Nurse in the State of Ohio
- CPR- ACLS required within 6 months of hire. PALS required within
in 1 year. (for AU-55081 only)
