*Per Visit Speech Therapist Lvl III*(br /((br /(*Middlesex Health*(br /((br /(*Middletown, CT*(br /((br /(*Per Diem*(br /((br /(Administers Speech and Language Therapy services under medical prescription in accordance with Hospital regulation, State of Connecticut Practice Act and quality standards.(br /((br /(*Key duties and Responsibilities: *(br /(* Develops a plan of care for each patient with the patient's input and agreement, which includes safety, educational, psychological and developmental needs of the patient.(br /((br /(* Interprets and assesses data about the patient as evidenced by appropriate goal setting and revision of goals throughout the plan care while explaining treatment goals, plans, techniques and outcomes to patient and or family.(br /((br /(* Displays expertise in recommending various therapeutic procedures and modalities.(br /((br /(* Complies with professional and departmental documentation standards.(br /((br /(* Addresses a home exercise program with each patient. Provides a written home exercise program and places a copy in the patient chart.(br /((br /(* Maintains adequate caseload, adhering to departmental productivity standards.(br /((br /(* Attends continuing education seminars as required by licensure and on return from such an event, schedules and presents an educational program including test to Rehab staff or other medical professionals.(br /((br /(* Participates in new program development, physician marketing and education of M.D's in the community about Rehab Services.(br /((br /(* Communicate regularly with Rehabilitation Supervisor, other team members and referring physicians for patient care.(br /((br /(* All other job related duties as assigned(br /((br /(*Minimum Requirements*:(br /(* Graduate of an accredited School of Speech-Language Pathology(br /((br /(* Licensed or eligible for licensure to practice in the State of Connecticut(br /((br /(*Preferred Skills: *(br /(* Experience with electronic medical charting(br /((br /(* 3 years clinical experience(br /((br /(* Flexibility with scheduling(br /((br /(Job Type: Part-time(br /((br /(Work Location: One location
